Type 2 diabetes is one of the most prevalent long term conditions in London, affecting about 4 - 7 per cent of the adult population. 

The MPS is a joint initiative, between Adult Social Care at Lewisham Council and NHS Lewisham CCG - in collaboration with providers and wider stakeholders.
