New online social care assessment forms go live

On 2 May Lewisham Council and its health and voluntary sector partners made a significant change to the way residents can contact and get support from care services in the borough.

Two new online assessment forms went live: one for adults who may be experiencing care difficulties and one for their carers.

By entering and submitting information about yourself you can get tailored advice and information about your health, wellbeing, care and support, and be advised of your likely eligibility for support.

For those who don't have access to the internet, they can still contact the teams by phone on 020 8314 7777.

This is the new number for adult social care, district nurses and Carers Lewisham services, which will be staffed Monday-Friday 9am - 5pm.
