There are two parts to your registration with LEAD. You will need to complete our:

1. Online Organisation and Information Form
2. Registration Form

The organisation and Information form allows us to place your organisation courses and information on the Website, while the Registration Form records your interest in being part of the programme.

Unfortunately, we cannot accept fax or email versions of the Registration Form as it must be signed and dated by you.
